Ronald Reagan leaves behind no biological grandchildren, although his adopted son, Michael, fathered two children, and his oldest daughter by Jane Wyman, Maureen, adopted a daughter from Uganda. His only brother, Neil, also didn't have any children:

Reagan first wife, Jane Wyman, gave birth to two children, Maureen and Christine:
Maureen Elizabeth Reagan (1941-2001), adopted a daughter, Rita Mirembe Revell, from Uganda, with her third husband, Dennis C. Revell.
Christine, a premature infant, lived for only a few hours after her birth (1947-1947).
Ronald and Jane also adopted a son, Michael Edward (b. 1945). Michael and his second wife, Colleen Sterns, have two children, a son, Cameron, and a daughter, Ashley.
Neither child born to Reagan and his second wife, Nancy Davis (b. 1921) has any children:
Patricia Ann or "Patti" (1952), who goes by her mother's maiden name of Davis, married a yoga instructor, Paul Grilley, in 1984; they divorced in 1990. Patti has no children.
Ronald Prescott, or "Ron" Reagan (1958), a liberal commentator, is married to Dona Palmieri, who is seven years older than he is. They have no children.
